• complete statements of declaration
• provide proof of professional development credits
• provide proof of liability insurance
• make renewal payment

Important things to note:

1. The renewal deadline is March 31. Late fees will apply.
2. Payment can be made:

• online by credit card at time of registration.
• complete credit card authorization form.
• mail a cheque or money order along your completed registration application, CE credits and proof of insurance.
